Terms & Conditions

Effective Date: September 17, 2026

Welcome to Elevate Medical Center (“Elevate,” “we,” “us,” or “our”). These Terms & Conditions (“Terms”) govern your use of the Elevate Medical Center website and your interactions with our practice, including appointment scheduling and communications.

Elevate Medical Center is a healthcare practice located in Kent, Washington, providing medical, wellness, and aesthetic services through appropriately licensed healthcare professionals.

By using our website, requesting an appointment, or receiving services from Elevate Medical Center, you acknowledge that you have read and understood these Terms.

These Terms do not replace any rights or protections you may have under applicable federal or Washington law.

1. Medical Services

1.1 Medical Evaluation

Medical, wellness, and aesthetic services are provided based on an individual assessment and the professional judgment of the treating provider.

Not every treatment is appropriate for every patient. Before treatment, your provider may review your medical history, medications, allergies, health conditions, treatment goals, and other relevant information.

Your provider may decline, postpone, modify, or discontinue a treatment when medically or legally appropriate.

1.2 Patient Information

You are responsible for providing information that is accurate and complete to the best of your knowledge.

You should inform Elevate Medical Center of relevant changes to:

  • Your medical history
  • Medications and supplements
  • Allergies or sensitivities
  • Pregnancy or breastfeeding status, when relevant
  • Recent procedures or treatments
  • Other health information that may affect your care

Providing incomplete or inaccurate information may affect treatment planning and may increase the risk of complications.

1.3 Informed Consent

Before applicable medical or aesthetic procedures, you will be given information about the proposed treatment, including relevant risks, benefits, alternatives, and expected outcomes.

Where required, you will be asked to review and sign a procedure-specific informed consent document before treatment.

Washington law recognizes specific requirements concerning informed consent. Procedure-specific consent documents are separate from these website Terms.

2. Appointments

Appointments may be requested through our website, by telephone, by email, or through other scheduling methods made available by Elevate Medical Center.

Certain appointments may require a deposit or other payment authorization at the time of booking. Any applicable deposit, cancellation, or scheduling requirements will be disclosed before or during the booking process.

You may also be asked to:

  • Complete required intake forms
  • Provide identification
  • Arrive before your scheduled appointment
  • Complete required consent or medical forms before treatment

Failure to complete required forms may result in a delayed or rescheduled appointment.

3. Cancellations, Rescheduling & Late Arrivals

Please provide at least 48 hours' notice when canceling or rescheduling an appointment whenever possible.

If you cancel with less than 48 hours' notice or do not attend your appointment, a deposit or applicable cancellation fee may be retained or charged according to the cancellation policy disclosed at booking.

Exceptions may be considered for documented emergencies or other circumstances at the practice's discretion.

Late arrival may reduce the time available for your appointment or require the appointment to be rescheduled.

Any cancellation or deposit policy will be communicated to you before it applies to your appointment.

4. Pricing & Payment

Prices for services are subject to change.

The applicable price for a service will be communicated before treatment whenever reasonably possible. Additional services or products will not be provided without appropriate authorization.

Unless another payment arrangement has been agreed upon in writing, payment is due at the time services are provided.

Elevate Medical Center may accept credit cards, debit cards, electronic payments, financing options, and other payment methods made available by the practice.

Third-party financing arrangements are subject to the terms of the applicable financing provider.

5. Refunds

Because medical and aesthetic services involve individualized professional evaluation and treatment, completed services are generally non-refundable.

Refund policies for products, memberships, packages, prepaid services, deposits, or promotional offers may differ and will be disclosed separately when applicable.

Nothing in these Terms is intended to waive or limit any refund, consumer, or other legal right that cannot lawfully be waived.

6. Treatment Results

Individual results vary.

Elevate Medical Center does not guarantee:

  • A particular medical or aesthetic outcome
  • A specific degree of improvement
  • A particular duration of results
  • That a treatment will produce the desired cosmetic outcome
  • That additional treatment will not be necessary

Results may be influenced by individual biology, health history, lifestyle, adherence to aftercare instructions, treatment area, and other factors.

Information on our website describing potential benefits or outcomes is provided for general informational purposes and should not be interpreted as a guarantee.

7. Risks & Treatment Alternatives

Medical and aesthetic procedures may involve risks, side effects, complications, or unexpected outcomes.

Before an applicable procedure, your provider will discuss relevant considerations and provide procedure-specific information and consent documentation.

Depending on the treatment, alternatives may include:

  • Another treatment approach
  • Additional evaluation
  • Delaying treatment
  • No treatment

You are encouraged to ask questions and discuss any concerns with your provider before proceeding.

8. No Emergency or Urgent Medical Care

The Elevate Medical Center website, email, text messaging, and other electronic communications are not intended for emergencies.

Do not use our website, email, or text messaging to seek emergency medical care.

If you believe you are experiencing a medical emergency, call 911 or seek immediate care at the nearest emergency department.

15. Reviews, Testimonials & User-Submitted Content

If you voluntarily submit a review, testimonial, photograph, video, comment, or other content to Elevate Medical Center, you should not include private medical information that you do not want publicly disclosed.

We may use voluntarily submitted content only in accordance with applicable authorization, privacy, advertising, and other legal requirements.

Nothing in these Terms authorizes Elevate Medical Center to publicly disclose identifiable patient health information without the authorization or other legal basis required by applicable law.

18. Limitation of Liability

To the extent permitted by applicable law, Elevate Medical Center will not be responsible for indirect, incidental, special, consequential, or punitive damages arising solely from your use of the website.

Nothing in these Terms limits or excludes liability that cannot legally be limited or excluded, including liability arising from professional negligence, medical malpractice, or violations of non-waivable statutory rights.

Any limitation relating to medical services applies only to the extent permitted by applicable Washington law.

19. Governing Law

These Terms are governed by the laws of the State of Washington, without regard to conflict-of-law principles, except where applicable law requires otherwise.

Any dispute will be handled in a court with proper jurisdiction and venue under applicable law.

Nothing in these Terms prevents a patient or consumer from exercising rights that cannot lawfully be waived.

20. Changes to These Terms

We may update these Terms from time to time to reflect changes in our services, website, business practices, or applicable legal requirements.

When we make material changes, we may update the effective date and provide additional notice when appropriate.

The updated Terms will be posted on this page.

21. Severability

If any provision of these Terms is determined to be invalid or unenforceable, the remaining provisions will remain in effect to the extent permitted by law.

22. No Waiver

Our failure to enforce a provision of these Terms does not constitute a waiver of our right to enforce that provision later.

23. Entire Agreement

These Terms, together with any applicable policies, notices, consent forms, payment terms, and other written agreements provided to you, govern your use of the website and applicable services.

If a procedure-specific consent form or other written agreement conflicts with these general website Terms regarding medical treatment, the more specific document will control for that subject matter, to the extent permitted by law.
